The treatment of blastomycosis with amphotericin B.

چکیده
Although antibiotic agents were introduced beginning in the mid-1930's, there was no really useful agent for the treatment of deepseated mycotic infections such as histoplasmosis, cryptococcosis, coccidioidomycosis, and blastomycosis until the introduction of amphotericin B in the late 1950's. The present report will review some of the available information concerning amphotericin B and will present our ideas about its use in the treatment of blastomycosis.

Guidelines for the treatment of blastomycosis are presented; these guidelines are the consensus opinion of an expert panel representing the National Institute of Allergy and Infectious Diseases Mycoses Study Group and the Infectious Diseases Society of America. متن کاملSuccessful treatment of cerebral blastomycosis with voriconazole.
Blastomycosis can occasionally involve the central nervous system (CNS). Amphotericin B deoxycholate is considered the drug of choice for the treatment of CNS blastomycosis. Significant toxicity may be associated with its use. We describe a case of cerebral blastomycoma that was successfully treated with voriconazole.

The lipid formulation of amphotericin B is the initial drug of choice for central nervous system blastomycosis, but it is costly and associated with significant toxicity. This case report details a patient with primary pulmonary blastomycosis with dissemination to the skin, one joint, and the brainstem that was successfully treated solely with high-dose fluconazole.
متن کاملBlastomycosis.
Blastomycosis is an endemic fungal infection due to Blastomyces dermatitidis that most commonly causes pneumonia; but the organism can disseminate to any organ system, most commonly the skin, bones/joints, and genitourinary tract. Both immunocompetent and immunocompromised persons can be infected, but more severe disease occurs in the immunocompromised. Blastomycosis can be diagnosed by culture...
متن کاملBlastomycosis of the central nervous system: a multicenter review of diagnosis and treatment in the modern era.
BACKGROUND Central nervous system (CNS) involvement with Blastomyces dermatitidis is an uncommon and potentially fatal complication of blastomycosis. METHODS We retrospectively reviewed 22 patients with CNS blastomycosis at our institutions from 1990 through 2008 (13 proven, 5 probable, and 4 possible cases). RESULTS Magnetic resonance imaging was used in most patients, alone or in addition...
